Immigration medical examinations performed outside the United States and its territories must be done by a panel physician. Panel physicians are different from civil surgeons. Panel physicians are designated by the Department of State and provide immigration medical examinations required as part of visa processing at a U.S. Embassy or consulate abroad

Why would there be a problem?  Historical earnings are a data point but not the threshold.  Current income is the key.

Worst case you get an NTA.

Have you inquired with the consulate regarding the sb1?  I would still exhaust that option all the way to refusal before going any other way

Maybe i am wrong, but my understanding is that the first step in the sb1 process is to make a consulate appointment and have the first of two interviews.   They won't have much choice as to answer questions when you are sitting in front of them.  You've been through it before so you have more familiarity than most

I understand its subjective but the pandemic was circumstances outside your control.  I suppose they could argue you had some control with all the  repatriation flights.

There is a section on the ds260 for OTHER NAMES USED.

You put the petition name in that section

Most panel physicians require proof of scheduled interview before doing the medical anyway

You obviously don't understand the full process.  Any immaterial error in the petition, such as misspellings, typos, etc.  can be (and should be) corrected when preparing the ds160 visa application that gets submitted to the consulate.

You need an officiant and one of the three parties must be present in Utah during the zoom event.   The officiant after the ceremony files the license with the county clerk

You can use them but you are under no obligation.  I could do it (if either the bride or groom was in Utah) and I would do it pro bono.
