I see that alot of vaccines are required for an immigrant? Will my wife have to take vaccines at her medical exam or can she decline? And will that cause them to deny her visa? She doesnt have a vaccination record. Would prefer for her to just get them in the states when she comes.

No, she can't decline. She needs to complete some vaccines to me able to get her visa. She can email the doctor and ask for more information (like I did).  I didn't had my vaccination records too so the doctor told me to take some specifics (I don't remember right now).

Why she can't take in her country? What is the problem?

There is a few she needs to have/take but it depends on her demographic; age, gender etc. There is a chart if you search for it. I also find it strange there are no vaccinations taken.. many are required as standard in most countries and given as routine during childhood.

The whole point of these checks is to stamp out risk of these diseases from being spread when vaccinations do just that... Surely this pandemic highlights the need for them?

At least one, MMR, is waived if your wife is expecting.

They test for a lot of things (a lot of people have already had chickenpox) but for the most part if there is not a valid medical reason for it unvaccinated = inadmissible.

There is a waiver, not that I suggest going that way.

If she doesn't have vaccine records she can do blood titers to show sufficient level of antibodies. Do it before the exam as it can be couple of days to get results back. Some vaccines are contraindicated in pregnancy so she won't have to take them (MMR). You're past the flu season so don't need that one either.

This, I was 42 at the time of the medical, I had no records as the health authority at home couldn't find records for me from the 70's. I had the titre test and just needed the Hep B shots, I'd already had chickenpox which showed up on the test too. It'll most likely be cheaper for her to get it done in her home country anyway.

She will be required to receive all of the vaccinations:

mumps, measles, rubella, polio, tetanus and diphtheria toxoids, pertussis, influenza, haemofilus influenzae type B (HIB), hepatitis A, hepatitis B, varicella, pneumococcal, rotavirus, meningococcal

She will not pass her interview without having these vaccinations.

In my husband's case (he is from Ukraine) he has received vaccinations as a child and youth in his home country but there is no physical record of them. When this is the case, you can talk about your situation to the doctor (embassy approved!) who you will meet with prior to your interview and he will do what is best for your case.

In our case, we are just starting to slowly go through the vaccinations now - this time with a paper trail - to avoid my husband having to get them all at once.

It depends on the local doctor, but the generally applicable CDC regulations do not require Hib/Hep A/Hep B/Meningococcal/Pneumococcal/Rotavirus for adults below 65. Since Rotavirus targets babies, the WHO does not recommend the vaccine for anyone above 2, so I seriously doubt your husband was required to get it now.

Required subject to applicability. The vast majority of those have very limited age restrictions to be required. The MMR ones do not apply due to a pregnancy.

I'm not sure what the issue is with getting everything in one visit.  Tdap, mmr, varicella (often times is waived), and seasonal flu.   That's it.

YMMV

Interesting. It might be by embassy/country. Here you need them all.
