I am slightly confused and wondered if anyone on here could help me?

I have received my interview letter and registered my appointment with the embassy in London using the Immigrant Visa website link. I have also paid the £20 fee to have the courier return docs to my home address.

I was under the impression that there was a further fee to pay (approx $200) for the actual greencard, or some other fee at this point. However I cannot find any info on this or any instructions on the Immigrant Visa login.

I think you're referring to the Immigrant fee $220 payable directly to the USCIS, you will get instructions on how to pay this once you successfully receive the visa, it will be a piece of paper bundled with your passport. It can be paid anytime after you received the visa, but must be paid so that the USCIS can print and send out your card. Most pay this before the fly into the US.

Yes! This is it. Thank you so much that’s really helpful.

Are you saying to reserve your spot for medical as soon as you are DQ'd and not wait for the interview letter? OR are you saying to reserve your spot for the medical when you are still in the NVC process?  I'm confused any feedback would be greatly appreciated.
