Check the actual instructions to the form. The instructions make no mention of this for the petition. Plenty of empirical and anecdotal evidence to prove that it is not a petition requirement.

Maybe. Get a hold of the IV unit of the embassy and see what they say. I am sure you will need to demonstrate a valid reason. If they agree you will need a new medical as the medical only had a 6 month shelf life. They may ask for other updates as well
