"K1 Fiance Visas are generally denied when the Petitioner fails to meet the requirements of a Fiance Visa, when the forms are incorrectly filled out and never corrected, our when there is a lack of evidence to support the claim of a genuine relationship.
There are several options available to you if your K1 Fiance Visa has been denied. But, before deciding upon a course of action, you must first determine if the denial was made at the USCIS stage or at the Consular level (Embassy).

If your case has already been returned to USCIS for further processing, you can USCIS to reaffirm their original approval and resubmit your application to the Embassy for a second time. Keep in mind, this process can take a very long time and for that reason, you may be best to simply withdraw your petition and then re-file."

sorry for the confusion---the date that still only matters is the date on the certificate itself---you won't have your citizenship revoked, regardless. 
With the passport issue, depends on the actual charge.

Actually from what I read on the Government website, immigrants have a 5 year waiting period after getting legal status (gc) before they can get Medicaid, so the fact you even got it surprises me unless I am missing something with your original post?

As mentioned, the tourist visa is fully unrelated to you. You don't sponsor someone for that.

Well, common sense - Legal residents of the US, need to keep their legal residency in the US...
 
If someone use their green card to "live" in the US only for few months every year, there is a BIG chance that eventually immigration officers will question that person and they will face the risk of loosing the green card.
 
If someone knows they will not live permanently in the US, then IMO they should NOT bother with acquiring green card.
