Need help missing vaccination for DS-3025 and Previous divorce record

Hi, my SO came to US (with K-1 visa) on Nov 9th and we got married on Nov30th.

I am currently preparing documents for AOS but had couple of questions:

 

She had her medical exam done before the interview but didn't get all the required vaccines. She was told she needs Measles, Hep B and Flu shot. We have her DS-3025 form that was emailed to her from the clinic.

 

Do we need to get those vaccines first and include them in the AOS packet?

Can we get those vaccines from any pharmacy or does it have to be from USCIS approved places?

 

Another question, do I include her divorce decree from her previous marriage in all applications (I-485, I-131, I-765)?

When you submit AOS packet (I hope you're filing I-130 together with other forms?) you can put all forms at the front all the evidence at the back. You don't need to repeat evidence. At least this is how my AOS was filed and got approved.

Logistically, and to avoid any issues with missing information or DS-3025 expiration, it may be easier to get a full medical exam in the US again. The doctor will issue all missing vaccines and ensure all the paperwork is up to date and up to USCIS standard.

The issue is needing to get the Vaccines signed off by a Civil Surgeon. They will need evidence that they have been done. Not all are willing to do just the vaccines.

K1 AOS applicants do not submit an I-130 unless they married after the 90 day authorized stay

As others mentioned, you'll need to get a medical done by a civil surgeon here (ie, I-693) since she didn't complete the requirements during her medical abroad. 

Yes, include a copy of the divorce decree. I just submitted and that's clearly spelled out in the instructions - and better to not assume they're going to look at previously submitted docs (unfort but lots of repetition in this process). 

And, which was also clarified, just second-ing that you should not submit I-130 if you married within the 90-day period.

I was initially confused after i-130 comment since none of the online guides or USCIS webpages mentioned it but thank you guys for clarification that I don't need it in my case.

Yes, you may get the vaccines from any pharmacy.  But the vaccination records need to be transcribed onto form I-693, which may be signed only by a USCIS-accredited civil surgeon.  You must submit the sealed I-693, not the incomplete DS-3025 that you have.

 

Some civil surgeons require that you repeat/pay for a full medical, to sign the I-693 for you.  You may need to call a few before you find one who will charge you only for transcribing vaccination records.  Search from this list -- https://www.uscis.gov/tools/find-a-civil-surgeon

No, just with the I-485 package as part of the evidence attached to that form, should be fine. The instructions for I-131 and I-765 don’t mention it and shouldn’t need it.

Little update:

Had to get the medical done again since nobody around here in Maryland want to do just the vaccination portion of the i-693.

 

 so got the sealed i-693 and sent in all the documents for AOS.

If the DS 3025 is not marked complete then you need to complete the vaccinations and get a civil surgeon to prepare an I693a that you will include with the i485.

 

You prepare the applications to be able to stand alone.  If mire than one application requires the same evidence then you will provide it multiple times.
