NVC:
Received NVC case number in response to a pubic inquiry (case was created on July 13, received the response to my inquiry with my case number on July 15. (Also, during the inquiries process with NVC trying to get our case status and number, we received conflicting information from NVC - eg, one response saying that had it and then another response a week later saying they didn't. But we didn't worry since that happened to others as well - they did have it in fact and just us an inaccurate update/template response.)
Eventually received our case number via public inquiry - Received on July 15. Used the formula to determine the case was actually created on July 13.
Petitioner received the FTP letter from NVC minutes after status changed to in Transit.

Submitted DS 160 and paid visa fees as soon as we saw it was “ready.” Was able to find the instructions for paying the fees by googling the Embassy’s packet three.

Embassy:
We would have booked an earlier interview if they were available but given it was in August when Italians go on leave. This was the earliest appt we could get after our NVC status changed to "ready."
Appears that Naples schedules, K-1 visa appointments, Tuesdays and Wednesdays. Makes sense because the medical is automatically scheduled for the day before.
