FAQs: Forms I-693 and DS-3025

I have my DS-3025 but need one more shot. I have a blank I-693 for the Civil Surgeon so all my vaccination info can be put into Part 2 of the form. So do I just submit that with my AOS application? My "main" medical examination stuff was submitted at POE in the sealed envelope. Does the Civil Surgeon need to fill out the entire form? Do I submit it half-filled? Very confused...

There is a lot of good information here, but I guess I'm a little confused when ti comes to our situation.

My finacee is coming on a K1 visa and will need her third HPV shot when she arrives. I assume that means she would need to have the I-693A filled out and singed by the local USCIS-approved civil surgeon. Is the I-693A sections 1, 2, and 5 of the I-693? I did not see any supplement on the USCIS website.

Does our situation (getting one more shot before sending in the I-693A) make sense?

I will reread but I assume the Form DS-2053 is issued by the USCIS-approved civil surgeon in her country (Brazil) in the sealed envelope and opened at the POE by immigration authorities. I believe she got a copy of this and all of the vaccinations (minus he 3rd HPV shot).

I assume the civil surgeon we go to can also give her the last HPV shot, but will call before. Is there a general cost for these services (I've read $100-$200 before)? She would hopefully be on my insurance plan by then so that might take care fo it.

I actually found out that the document on the CDC website is not updated. According to the USCIS instructions to I-693, Part 1 and the vaccination supplement for Part 2 needs to be filled out, and Part 5 must be signed by the USCIS-approved civil surgeon.

The vaccination supplement now has sections for Rotavirus, Hepatitis A, Meningococcal, HPV, and Zoster, whereas they were not required before.

I will have to check and see with the doctor if all 3 shots of HPV need to be taken before we can submit the AOS. I'm afraid we will have to...

Alright I read many posts about the Ds3025 Issue and the I693A issue....

(PS: I might have mixed up DS3025 and I693 forms... Well I don't really know which is which -_- )

Now please correct if I'm wrong! ...

1) Since I am a K1 applicant, I read that I don't need to give in an I693A but to only send in the DS3025 (vaccination worksheet?)

2) My DS3025 was done in Canada and on the paper it said "Use in Canada only" so that's why people like me need a civil surgeon to fill out an I693A for us right?

3) I saw many posts about how people did not send in their I693A and got their AOS approved (they didn't get any RFE!!!)

4/07 - Filed the I-129 F [Petition for Alien Fiancé(e)] Along with Bio forms [G-325a for both of us]

6/07 - Forms got resent back to my Fiance because I forgot to sign some papers. [silly me]

11/28/07 - Got my K1-Packet 3!!! It was a headache to fill in the papers.

2/15/08 - Sent all documents and forms through express! I was close to the expiration date

3/27/08 - Interview letter!!! I was in the US with my Fiance so I couldn't come back in time, rescheduled appointment.

4/24/08 - Got my interview done :) Approved! Passport+visa+brown envelope will be sent within a week.

5/1/08 - Got my Visa!

~ Flying to US on 6/11/08.

~June 19 2008- Married!!!

7/25/08 - Sent in AOS: I-485 along with EAD, AP.

8/7/08 - EAD, AP, AOS touched.

8/19/08 - Got an RFE for I-485... Fixing it.

8/22/08 - Biometrics taken in Portland.

10/08 - Another Bio (for work authorization card).

-- received my work authorization card and advance parole papers in meantime--

1/15/09 - AOS INTERVIEW *faints* in Seattle, Washington.

2/2/09 - Green Card!

Q: Why are K non-immigrant visa holders asked to include a vaccination supplement when filing Form I-485 to adjust status?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include the I-693a vaccination supplement in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693A IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693a.

In light of the recent changes (i.e. discontinued 693a, new TB policy?), does the statement in red still hold true?

I am trying to figure out that one as well since my fiancee has one HPV shot left to take here in august but I wanted to see if we could send in either the DS-3025 alone or if its necessary to send in the I-693 supplement. In the case of the latter, do we need to wait until the 3rd shot is taken or can we go ahead and send it in right after we marry next month?

I read on the USCIS website that you don't have to complete a series of vaccinations to file for AOS. As long as the first dose is administered, you're good to go. It's your responsibility to make sure you get the other doses. They don't require proof that they have been given.

My wife completed her vaccinations before getting the k-1 visa in April. We're just sending a copy of her DS-3025 and a note stating that she completed her vaccinations as part of the k-1 visa process. If I get an RFE, so be it. We're in no hurry. With all these recent changes, nobody knows what the hell is going on, so we'll just see what happens.

It's good to know we're not the only ones going about in semi-blind. I guess we'll just send in the DS-3025 even though I've read the I-694 Parts 1 & 2 (supplement only) and 5 are necessary and hope that' all we need. Later, we'll complete her 3rd HPV shot for her own protection.

Please let me know if anyone disagrees with what we are proposing.
