For you folks in the Los Angeles area, especially the South Bay, I can report that my experience with this office was great. The front office staff was very easy and pleasant to deal with. I simply dropped by their office on Friday, June 25, 2010, filled out part 1 of the I-693 and a couple informational forms, let them xerox my DS-3025,and paid a $50 fee. On Tuesday, June 29, 2010 I stopped by their office and picked up the sealed envelope for the USCIS and an envelope that contained a copy of the I-693 for me. That was it. Easy cheesy. :D

A tip for anyone who's looking for a civil surgeon: if they don't understand your request (to merely get your vaccinations transcribed to the I-693) on the phone, then it's unlikely they'll understand in person, and they'll almost certainly try to get you to do the physical and/or vaccinations again.

I live in Philadelphia Metro Area and I called almost every CS today. After speaking with all of them the only viable choice was a CS that is charging a whooping $75 JUST for transcribing the vaccinations onto I-693 (our vaccination record is complete).

Everyone else wanted to do a full exam or wanted over a $100 for transcribing.

This is a review of Civil Surgeon for NYC

I called in advance after reading for hours how tricky (abusive) some of the Civil Surgeons were and decided that my best bet was Dr. Noel Smith

I had the chance to speak with one of his secretaries few days in advance to my appointment. He told me that if I had all my vaccinations up to date (I made sure to take HIV, STDs and TB tests at the local Fort Greene Clinic in NYC, cost zero to me), I will have to pay $75. That was a relief from what other Dr. in Brooklyn wanted to charge me, $175.

So in the day of my appointment I was welcomed by PAOLA (Dr. Smith's secretary) with a warm smile and she helped me and my wife true the process of getting all the necessary info so Dr. Smith would have to only sign and approve my application.

It didnt take more than 30 minutes and $50 for the Dr. Smith's team to put my package all together.

I am very thankful to Paola, and Dr. Smith for all the great help.

I can only recommend them.

After an exhaustive search, and being told by several offices that they will only do a full medical, and hearing quotes as high as $120 - I found this doctor in San Diego. They knew exactly what I needed and were friendly and professional. The entire appointment took about 10 minutes. Cost was $30

I did have to get a booster shot of MMR, even though what I had was fine for the US doctor in Toronto. I was told that the last shot was to be no longer than 10 years ago. If your last shot was over 10 years ago, I would suggest getting another one. In my case, I had to go to another clinic and pay for the shot, which was only $10 anyway, so no big deal.

We didn't actually see the doctor as he was very busy and was examined by a different doctor (His signature is on the papers though). However, the center staff were very friendly and professional, and they definitely knew what to do, even the receptionists. We called to make an appointment beforehand, and they knew immediately what to do. Even though they were aware that K1 holders do not require a medical, they said the doctor usually goes by a case-to-case basis (But they also said that most people didn't need it). I was registered as a patient because I required the tetanus shot, so the actual transcription cost was $15 and the shot/consultation fee was $48. If you do not require any shots, it will be $30. They mailed 2 copies to us (one for USCIS, one for our personal records) within a week. All in all, I would definitely recommend this place.

Note: I am a student and visited my student health center for booster MMR and Td shots. Also did my bloodwork and TST test at the health center, which cost me less than getting done at his place. I suggest you do the same, or visit a free health clinic to get them done prior. Having options are always better.

There are three civil surgeons listed in Tallahassee, FL and this is the information I got:

They are charging $175 for transcription + bloodwork + TB skin test. (Any other vaccines you will need they will charge extra) Even if you have completed the tests within the last 12 months, their base fee is fixed at $175 per person.
