I highly recommend this office for those who live nearby although we traveled an hour to get there. They are so professional and so nice! I had to explain my situation again and again, as they didn't understand the first time. Hey, I'm fluent in English, but I guess I omitted something. You have to specifically say that you need to have your form DS-3025 transcribed on form I-693, but only Part 1(info about you, which you have to complete prior to the appointment), Part 2 - only the VACCINATION CHART(as it is stated on USCIS website for K nonimmigrants) and Part 5, the CS certification.

Make sure to explain as best as you can. I did great on the phone, but while I was there at their office, I forgot to add "the vaccination chart", when I said I need Part 2 completed. So they misunderstood me and thought I wanted them to transcribe something for which I don't have proof(the whole medical exam done overseas). Silly me, haha. So yeah, try to emphasize as best as you can that you only need those parts completed and specifically Part 2-vaccination chart.

Standard cost: $40. I didn't need any additional shots.

Waiting time: About 15 min before being called. Then I wasted more time explaining, but finally I understood what I said wrong and solved the problem. I got the sealed envelope in about 5 minutes, so it was well worth it!

Just used doctor Bukari, nice guy explained to him the issue. Charged $50 dollars to transcribe the vaccinations (Usually charges $100) and said my wife was due for a second shot of (3 Months later) of varicella.

After calling every number from the USCIS website of doctors in my area, I am happy I went with Akron's Concentra. The receptionist I spoke to was very helpful and even called me back to confirm the cost. There was a mixup at the time of my interview (they initially thought I needed a physical exam) but the doctor cleared that up and I wasn't charged for the exam. Dr. Benson was friendly and was happy to fill out the paperwork and have a chat about immigration. :)

Beware of the Concentra in Cleveland though. When I called them I had a very unpleasant talk with the receptionist who told me that they "don't do AoS" and that they ONLY do physical exams for immigration, then she hung up on me.

Best civil surgeon in NYC!

I went to Dr. Anthony D Blau, Green Card Medical Exams yesterday. They know what they have to do as a civil surgeon!!! We didn't need to explain anything about the I-693 form and the vaccination supplement. I have done all vaccination before coming to the US, and I brought my vaccination history to the clinic so mine went very easy. I paid $60 for the vaccination supplement. The doctor and the lady at the front desk were really nice :)

We tried calling other civil surgeons in NYC including Dr Smith. Most of them only offer a full medical exam. Dr Smith charges way more than $50 for the vaccination supplement now.

Well I feel I MUST share today's CS experience. At first I was calling left and right to find one who will do transcription only, and I got plenty with attitudes. I called those in Rialto, Riverside and San Bernardino and they said full physical, that is all. it left me with a really bad attitude towards the CS folks.

I used this thread to find one in Torrance, but that is quite a drive. I saw another in Chino, so I called that one. I was told by the receptionist that they only did physicals, then I mentioned this VJ thread and asked, "Hmmm you are listed as one of the places who will NOT rip us off." She handed me over to the office manager who said they will do transcriptions and they wanted to know about this website thread. I told them all about it and my appointment was for this afternoon to get the paperwork done.

First thing this morning my fiance needed two more shots - Varicella and MMR. For the record, the MMR hurts like the dickens. The Varicella was $140 and MMR was $80, total of $220. the place looked rather ghetto too, you need shots just to go to that part of Riverside,haha. Then we headed out to the CS with our completed shot records for my fiance and two kids.

What a great experience! The office is very clean and bright, the staff is very friendly, and there was hardly any waiting. Isabel told us if we wanted a full physical it would be $110 and they will do ALL shots for another $110. So for $220 we could have just come here in the first place, and they will do the ENTIRE series of shots! They were more than happy to do the transcription, they checked and rechecked the records, and then at no extra charge took basic vital signs and opened a medical record in case we visit again. I am seriously looking at making them our primary physician once I get everyone on my insurance.
