I had a great experience with Dr. Edna Butaney in Boston. After reading online about some awful experiences with some of the other civil surgeons in the area, I was a bit nervous. I had to have the whole medical exam for the I-693 done. I already had all the required shots (if you go after October 1 you need a flu shot). Since my PPD had been positive in the past and I had a proof of that, she didn't do it again and just sent me to get a chest x-ray. All in all, very friendly and well informed about the process, paperwork was ready very quickly. Fee was $180.

I couldn't find anything for Jacksonville, FL so I wanted to contribute to help future people.

I called a handful of places and the doctor either didn't do it anymore, or wanted to do a full exam for $150. Way too much.

Finally found one that would just do the transcription for $50.

Just wanted to add a recommendation for Dr. Steinberg. She was very nice and very fair. She charged $30 for the transcription and $60 for two vaccinations.

We previously called another CS in our area. He wanted $120 for the transcription and over $300 to include the vaccinations. Crazy!

To update an old listing:

Dr. Fernandez in Garner, NC was fantastic for our K1 AOS transcription. I saw him listed here a few years ago. The price has gone up slightly for transcription-- to $35-- but the staff was knowledgeable and polite, it was quick, and they knew what they were doing. There was no confusion or trying to up-charge for a visit. We were in and out in under 30 minutes. The location is quite convenient to the Triangle-- a few minutes south of Downtown Raleigh.

I think it is normal, but a warning: we didn't have the varicella vaccine on the original physical in-country, so we had to either get the vaccination ($140-ish) or the blood test ($50ish) for it to be completed. With the vaccine, we got it same-day. With the blood test, you had to fill a medical records request/release form and then they would have mailed it to us within the week.

BE CAREFUL. They are way too old to be professional doctors.

I went to see Dr.Noel L Smith in NYC last week and it was a really bad experience.

The office and the rooms are really messy. Even the shots or medicine are not organized. It's really scary to get a shot in such a dirty room.

In the beginning both of the doctors misunderstood my medical record and had me have a bloodtest which wasn't even neccessary. (I already had one in my country 2 month ago.)

They realized it after they took my blood. When I asked the refund, Dr. Smith(a male one) handed me $20 from his pocket. which wasn't enough ofcourse.

They also didn't noticed that I was missing one vaccine shot untl I pointed at my vaccine record. 5 minutes later, Dr. Smith(the female one) forgot to do it.

In the end, they even handed me someone else's report.

Everything was wrong. I know some people on VJ recommended them before but I guess they can't read or understand the things anymore.

Trust me, you really don't want to go there. I just don't want anyone to feel the same as I&my husband did.

We just walked in, asked for the transcription service. Only took 3 days to pick-up paperwork in sealed envelope. Funny, when the lady said $15, we though she said $50. So my husband handed her a $50 bill. Then she repeated "FIFTEEN"...even better!

Hi, We went to a Civil Surgeon on ( 2 / 29 / 12 ) and were told that my DS-3025 looked fine and the only vaccine needed was the TD. He transferred all info from the DS-3025 to the I-693. The price was $77.00. The office sealed the forms in an envelope and wrote " DO NOT OPEN FOR USCIS USE ONLY - CONFIDENIAL " on it. We received copies of the I-693 for our records. Also the doctor ask if Hany had received a chest X ray in Egypt at the time of my medical exam, Hany said yes I did , he then told Hany that he wasn't required to have a TB Test here in the states since the X ray was taken. Dr.Edwards was very polite and knowledgeable about the AOS medical requirements. Remember to take your DS-3025, Passport and Social Security Card with you. When I called for an appointment the lady on the phone quoted me from $150.00- $450.00 but as long as you have your DS-3025 the doctor will be able to tranfer most info from it and it won't cost near that much. Comments: They offer the full exam, but also understand the vaccine-supp requirements for those filing within the year. I was able to get an appointment within a couple of days and had no issue with them filling out the form. If you do require additional vaccines, they do offer them there, but it's good to note that the local health department offers vaccines at a much cheaper cost and with no appointment required.
