Daniela, Mrs.-T-B.-to-be had her clinic in Ecu construct her record in a one-page document on clinic letterhead. Shown were the vaccines given, the exact or approximate dates (from clinic files or her best recollection), and the clinic's stamp with someone's signature from there. She had two originals of this made, in case one copy got lost. When she arrived in the U.S., we carried this record to our county health clinic. She needed and got two additional shots (tetanus and something else), and the clerk hand-wrote these on the bottom of the Ecu document with her own stamp and signature. The shots cost something like $20 total, by the way. Accordingly, I suggest that you visit your usual clinic or one where they know you at least slightly, tell them what you want, and use a combination of clinic records and your best memory to achieve the above.
