I am in the same situation! I will still send the ones with my maiden name on them, but just put an explanation in the cover letter of the package that some items have my maiden name on it, while others have my married name.  I am sure that this is a common thing for them to see. ☺️

Yes.  I would include statements and would also provide them an explanation with your wife's name being used before and after name change.

Hi, we interviewed in Philadelphia, we took only the documents listed on the appointment letter, at the interview we were asked only for bank statements (joint account) and letter from insurance company certifying that my husband added me to his policy. Nothing else.

My Fiancee now has her medical scheduled for 8/16/2018 and just scheduled her interview in Tokyo on 10/1/2018 which is their first available appointment.  Needless to say but we will checking often to see if an earlier appointment opens up.

Still without case number 😔 I wonder why it's taking so long. I've been calling almost every day but nothing. Gonna try again tonight.
