Page 6,  Part2

information about your beneficiary

(About your parents)

•my father had divorced in long time ago.

(I don’t know he still alive or not)

•my mother; she abused me, so ran away from her, and I haven’t seen her for over 16 years.

Name them both with their date of birth if u know it

put N/a for address

The USC  can add a blank page to explain that u do not know this information

but u are the beneficary and u say this is about the bene parents

and the form is for the USC to  fill out and he/she should follow instructions for the form

In addition to that, even if its their first name you remember........just put it in there , use the part 8 attachment to offer explanation ....its no big deal...Even if you from the orphanage

Thank you answer the question.

so it’s okay “N/A” in there, isn’t it?

well, I’m beneficiary, and my parents like that.

or should explain on page 13(additional information) ??

Again,  the USC is suppose to fill out the 129 form as he is petitioner and needs to sign

but, yes,  put N/A and explain

Fill out as much information as possible.  Name and date of birth.  Put n/a if you don't know their physical addresses.
