Comments: They offer the full exam, but also understand the vaccine-supp requirements for those filing within the year. I was able to get an appointment within a couple of days and had no issue with them filling out the form. If you do require additional vaccines, they do offer them there, but it's good to note that the local health department offers vaccines at a much cheaper cost and with no appointment required.
