The swab test in philippines are doctored up to keep people there, my wife has no symptons always wheres a mask and they say she is positive,  what can we do to get visa extended?

Sounds like a false positive (unless she's asymptomatic).  Is there an option to take another test?

Remember that not all COVID infected show symptoms or become ill. Try contacting embassy but i doubt the visa can be extended. Your wife may need to do medical again and show up for interview again (pay visa fee!! Not sure) if the visa expires by the time it shows -ve result.

You only need to quarantine 10 days, you need 6 months????  Why do you need a visa extension?

Yup.  Just edited my post.  It's already the 4th here in Manila.  Forgot that it's not over there.

No way to extend.  If it expires, the consulate can, at their discretion, re-issue.  If the medical expires, she will require a new one, and possibly another interview.  Contact the consulate.

They gave us only 5 months on medical instead of 6 months

If medical expires does that also expire the visa

For some unknown reason, my wife's visa's expiration date was only 5 months after the medical also.  I never found an explanation.

Yes.  The visa expiration is tied to the physical exam date.
