Yes. In the same envelope, since its a concurrent filing (i.e. also include I-765 and I-131 in this envelope).

OP I would also include the I-131 - there is no additional filing fee. This will allow the applicant to receive an advance parole card to be able to travel out of the country before the GC is received (likely to be in excess of 12 months). This is probably important so that the applicant can return home to gather belongings, sell a car, return keys, clear out the apartment, close the bank account, switch off the utilities, and finalize other such things that were obviously not taken care of beforehand.

There is a fee calculator on their website. Also it has been added to the guides. Probably it is not visible enough....

right - so they'd need to marry prior to ESTA expiring (90 days). Filing paperwork can be done later, as overstay is forgiven in that scenario.

No such requirement exists except for K-1 visa holders who wish to AOS based on the existing approved I-129F.
