Yes. Oddly enough, considering what this form is for, I couldn't type in the name of my employer or my job title. I had to print it out and fill these in by hand.

Oh I see..so I guess all the pages filled in by hand should be scanned then..hope that works🙄

Timeline

For anyone who is using the travel docs embassy specific website to schedule your appointment, is there a likelihood for earlier appointments to open up based on cancellations? Last week I wasn’t able to schedule an appointment because our status wasn’t ready but I could see the next available appointment was April 10 since then I’ve been checking everyday and when I finally could schedule it the next available appointment jumped from April 10 to May 6! I scheduled it just in case but I was really hoping we’d get an earlier appointment. The website allows you to reschedule but I’m not sure what the likelihood of an earlier appointment opening up is, has anyone seen this happen?

@Lou&Don Many of our answers are predicated on the information you supply in your profile. Yes it does happen where an earlier appointment opens. But be aware that some Embassy's will penalize you for excessive cancellations. Most use the 3 strikes rule. If you go pass your allotment of cancellations then you are sent to the back of the interview line and must wait 90 days before  you are allowed to schedule a new date but only after paying another interview fee.

Thank you Greenbaum! I will only reschedule it one time and only  if I find an April appointment like I was expecting. I didn’t think they’d skip 20 days of appointments and jump into a new month. ☹️. If I learned one thing in this process don’t assume or expect anything.

I replied to this a little while ago, but my reply either disappeared or I didn't submit it properly. Sorry if it's a duplicate. You have to scan it anyway, to sigh it. I scanned the entire, completed document, filled it in manually wherever it was needed, signed it, and then scanned and saved the whole thing. It works.

I went to Ask NVC page to update my email address and ask for case number a week ago or so and I got this reply. Not sure if they received my case from USCIS or not? Anyone got this response before? My NOA2 was Feb 21st, it's been 4 weeks now.

Hi. My NOA2 was on Feb 4, 2019 but NVC only received my case on March 19th. We are still waiting for our case number until now. :)

Not yet! Our status still says, "In Transit", but we're hoping that will change soon. According to DHL.com a package left on March 19 (the same day ours left) and wasdelivered and signed for on Friday, March 22. Of course, we're hoping it's ours. I know there is at least one other couple whose package was shipped to Sydney on the same day and DHL has listed only one piece, so who knows! This has been such a guessing game from the start!

How did you check it at DHL.com? I tried different combinations but nothing worked🤷‍♀️

At DHL.com, hover your cursor over "Express"  then click "track by shipper's reference"   I believe yours was also shipped on March 19, so for "shipper's reference" put EXP 19 MAR 2019A. Then select a date range. I would put  March 18 as the "from" date, and leave today's date in the "to" field. Leave the account number field blank, and select your country. Click "Track", and hopefully, you'll see what was shipped! Even more hopefully, you'll see that it's reached your embassy! Good luck! Please, keep me posted!

Thank you! It worked! I forgot to put the EXP thing)

Says it's at delivery facility in my country already from Saturday, but i guess it wasn't delivered to the Embassy yet.
