This form: http://www.uscis.gov/files/form/i-693instr.pdf page 4, Qn 2 states that as I've already done my medical overseas, and my vaccinations were completed overseas I don't need to worry about filling in form I-693. HOWEVER, if I needed to have further shots AFTER my medical (the supplement) then I do need to have a civil surgeon transcribe it.

This form is a joke.... I have had my medical done in London and I just need the Varicella and Gardisil shots, spent the past hour calling everyone in a 50 mile radius and no one will do JUST the vaccinations, I am so mad as I don't have any money for a full medical, and they just keep trying to con me out of it.

I really want to complain to the USCIS but I think they probably already know about the problem and will not do anything about it. I am in Port St Lucie and if anyone knows of a medical office near this that will JUST do vaccinations and fill out the sheet then that would be awesome, I am waiting on the one doctor in PSL to get back to me as she only works certain days.

If I could get my vaccinations done in the West Palm Beach Veterans hospital that would be awesome (Husband is ex Navy) but I somehow don't think that could happen.

That sounds right to me. After all, if they're just taking your vaccination record and transcribing it, it shouldn't matter what doctor performed the vaccinations: whether abroad or around the corner.

Yeah thats what I have been saying to them all morning, the last call I told them a USCIS rep gave me their number and they can do vaccinations without a medical. And they told me "No we cant" so I was like "Fine then I will inform the USCIS that you don't offer this service".

We are just on the military helpline while I am nearly in Tears over stress from this. My Fiancee is trying to calm me down as I have been spoken down to all morning by stupid secretaries. One of them even put down the phone on me!

LOL! We should make a topic of "Who not to call for vaccinations". That would make my life and others so much easier. And hey! about 20% of florida I have already called would be on that list ;), saves people a lot of time....

Sorry guys I just needed to rant, I usually am pretty calm but I feel like I am about to explode.

Just got off the phone from USCIS, VERY interesting.

She said she couldent find any people in Port st lucie....Who do the medicals. I actually called the USCIS helpline (Automated one) and it told me there were 12 within my vincitiy. My fiancee just got off the phone and the lady said there were NONE in here database near us, just ones in West palm.... Which is slightly shady... Little bit confused but it makes more sense.

You might want to try printing out the instructions for the i-693 for the nurse. There's a spot in there about K-1 visa holders. The second paragraph in that section is what I printing out for my CS. Hopefully, mine and yours will figure it out from that. :)

You might want to try printing out the instructions for the i-693 for the nurse. There's a spot in there about K-1 visa holders. The second paragraph in that section is what I printing out for my CS. Hopefully, mine and yours will figure it out from that. :)

So I have printed out the I-693 instructions, I only have a copy of my Xrays and my vaccination records from the medical. Is that going to be enough for them to fill out everything without conning me in to another medical.

Be wary of this special instruction for the I-693. After January 4, 2010 the HIV test is no longer required.

HIV Testing: Starting January 4, 2010, an individual should no longer be tested for HIV infection for purposes of the immigration medical examination. 74 Fed. Reg. 56547 (Nov. 2, 2009). Please disregard any reference in Form I-693 and its instructions to HIV on or after January 4, 2010. When completing Forms I-693 on or after January 4, 2010, either:

* write "No longer required" in the entry for HIV testing in Form I-693, section 2.2.©; or

* leave the HIV testing section in Form I-693, Part 2.2.© blank.

We didn't know about this and we went and had Cory made the HIV test. So be wary of this new instruction.

Q: Why are K non-immigrant visa holders asked to include a vaccination supplement when filing Form I-485 to adjust status?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

Q: Do I need to complete another full medical to adjust status?

Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status.

Q: I received an RFE for Form I-693 and my medical is less than one year old.

Sometimes the medical gets "lost" during the transfer. It is entirely possible that you may receive an RFE for the vaccination supplement (Form I-693), and possibly even for the full medical (Form I-693), even if the medical is current. Here is a helpful post from someone who was able to relocate a lost medical.

To find a Civil Surgeon in your area you can link here to use the USCIS Civil Surgeons Locator.

To have your vaccination information transcribed to Form I-693, the following Civil Surgeon will do it by phone and fax for a nominal fee ($10 at time of writing):

Something to note if you are yet to have your medical: If you have not yet had your medical prior to visa issuance, make sure to ask the Panel Physician if they will supply you with a copy of your full medical prior to scheduling. If you cannot find a Panel Physician who will provide you with a copy of your medical, or if you have already had your medical and were not supplied with a copy, try asking the officer for one at the POE.

FYI:

When I had my medical at Woking Medical Clinic in Vancouver, Canada, on April 29, 2009, they would not give me a copy of my medical, but what happened was this:

When the Vancouver consulate gave me my big sealed envelope, my medical results were in a smaller, also sealed, envelope stapled to the big envelope. The clerk at the consulate who gave me my passport and packet told me not to open either envelope. He also told me "The smaller envelope contains three copies of your medical. Make sure you get one of them back from the customs officer when you enter the US, after they open it."

I think the USCIS need to send out guidelines to all the physicians.

They made me have Varicella (Have had chicken pox AND shingles! But hey! Who cares...Lets just stab Katie in the Arm and make her pay more!), and also another Tetanus... My last Tetanus was in 2005 (And they are good for 10 yrs...) and the Dr made me go to get another one at the local health clinic. Needless to say I am pissed off beyond belief and $130 later, one very swollen arm and a angry mother in law, I seem to have got nothing good from it.

I cant believe I had to have both shots... The Civil Surgeon didnt even read my vaccination record properly, just sent me on my way to that awful clinic. Where they pretty much told me they didnt want to give them to me, and I had to force them to... I still have to go back to the CS to get my vaccinations sent off. Sent in my AOS without the medical due to them loosing it so often recently.
