want to update.. just got noa2 this morning. thanks for your help guys

I finally received my NOA2 after replying to the RFE two months ago!   Mine took longer than other people, on average, but still got there.   Big monkey off my shoulders.

Sorry for the delay in updating.
I did indeed require a notarized letter, but had absolutely no way of getting it before my appointment. I really wish I had listened to my gut sooner.
I was declined for my visa, but all I needed to do was send in the notarized permission form as well as a photocopy of my ex's passport, which I had no idea I needed, as it wasn't mentioned here either.
I've already sent in the required documents and the consulate has received it, now I just need to wait (hopefully not too long, I'm pregnant and will be strongly discouraged from any travel starting in February.)
I did mention to the interviewer that I wasn't able to find any information about what I needed for my daughter and they admitted it was a common problem and that everyone there is aware of it, but it just takes a long time for any of the directions to get updated.

I agree with that!

Boarding passes are much better evidence than itineraries.

I have noticed the long layovers when routing through LAX.  For about $100 more you can route through San Francisco and the layovers are much shorter.  Maybe too short.  Good to hear you had no problems with Cheap-O-Air.  Thanks.

I hate to say this, I went with my fiance to Saigon for a vacation while waiting for the K1 interview.   She is from China, I am from LA USA.  We both have stamps in our passports which we mentioned and which CO looked during the interview in Guangzhou China.
 
So missing the stamp on the passport could be the accident and not the norm.

I did request FOIA few years ago and it only showed me left and entered US only. If you use loose leaf visa, they will only stamp on the leaf visa. That is the reason I use passport visa only. They stamped on my passport and sometime stamped my exit ticket as well.

A friend of mine recently had her k1 petition approved they got the visa and her fiance then now her husband are living together her in the states... When I was about filling k1 for my fiance she gave me an exact copy of their petition to go through so I have an idea what to submit.... I found all kinds of misspellings, dates, typo etc she even misspellt his name a couple of times... But they got approved... These small things don't matter expecially when they see it as what it is, a simple mistake... So don't worry, at this stage after submission we all worry maybe we did something wrong or didn't include something... Best thing to do is to invest time in your relationship with significant other because in the end that is what matters. 

I have to add though, I don't think that'll be a denial for sure. You still have these other stuff, boarding passes, ticket, receipts and stuff. I think it'll be alright. And just simply explain that you lost the Vietnamese visa. 

I don't know what to tell ya. They did not do that to my kid. Few years ago, when I took my son there, he had the loose-leaf visa and I thought they stamped the passport. That was the only time he had the loose visa. The next several time we did the visa exemption book. 
 
And they always stamped my passport because I always use my Vietnamese passport to enter Vietnam, and the US passport to enter the US. 

No big deal. If they ask, you say you messed up the date because it easily messed up, since you leave on Feb 6th but it's actually 7th over there and arrived on the 8th , but actually only the 7th here.... 

No.  Did you send passport stamps?

not a big deal, unless it was in a previous YEAR that puts you out of the 2 year rule.

Checked awhile ago and saw it! They must’ve approved it really late!

i can't say for sure. but both updates in my case so far have been updated on both websites at exactly 1:00 AM ET. i know because they always send an e-mail at exactly that time
