US Immigration from Philippines

Has anyone done a medical before and did not have any immunization record to bring with them? If so, what happened? I am going to have my medical soon and I do not have any immunization record to bring (except my COVID vaccine card). The US Dept of State website have this statement (below), so I am assuming it's ok to show up to the medical without the immunization record. However, I still wanted to come here and ask if anyone has experienced this. Thank you all.

"Visa applicants should consult with their regular health care provider to obtain a copy of their immunization record, if one is available. If you do not have a vaccination record, the panel physician will work with you to determine which vaccinations you may need to meet the requirement."

Applied for our ROM on 12/3/2021 after marrying online. The San Francisco Consulate reported it now takes up to 12 weeks to complete.
I thought I would receive an official notice that it was in-process and I could apply for the 9(a), but never did.
I sent an email and they responded saying to go ahead and apply.
I applied for the 9(a) on 1/11/2022.
Curious to hear from those who have received a 9(a) while their ROM was in process...how long did you wait for it to be approved?

NVC payment stage for my case, IR2. Paid the fees but the AOS was rejected. We used the same bank account info and it has sufficient funds. Has anyone experienced this before? What to do?

I am now finally starting to apply my Citizenship and I am confused about what stated on the NOTE: Select the box for either Item A. or B. in Item Number 1. If applicable, select the box for item Number 2.

I did clicked box A- I can read and understand English, and I have read and understand every question and instruction on this application and my answer to every question.

It means I don't need to click Item number 2?
