After being schooled, I googled DS-160 PDF. Sure enough, to the right of the question about relatives there is a little help note that says fiance, spouse, son daughter, etc.

The old paper application did include the actual word "fiance" in the list of relatives and an applicant was asked about each. Based on the DS160 and her circumstances, I would consider this a non-issue.  If asked.  Explain just the way you did here.

I assume the travel agent identified herself/himself as the preparer on the application? Your wife will need to explain that the preparer asked the question without elaboration as to the USCIS definition of relative and she understood the question to be asking about blood or by-marriage relatives only, as one usually would.

Not material, especially given that the visa was denied anyway.

Now you know for the future. Disclose it as necessary in the future, including in any interview. I doubt it would have any negative impact for a spousal visa.
