you can find there samples form online. But in ceac website they give you option to print those after completing before submit at the review steps

My mom finally received an appointment letter this morning for my little sister.  Still waiting on the interview letter for my little brother.  Kingston Jamaica.

 

F22 category and the interview is for February 20 at 7am

 I got DQ Dec 18th 2019 and IL January 14 for Febuary 20th interview. Lagos only send out dates once a month as I have notice and read here. Check your email for updates and if no email yet, that means you might have to wait till next month for IL for March interview.  I am not not 100% sure how it works though, I am sure @dwheels76 can help you out with the right answer to this question. Also you can check the spreadsheet.
