I've been agonizing about this for awhile now

Our attempt to find reasonably priced civil surgeon in San Diego area and were unsuccessful locally. Some listed on USCIS website don't even do those anymore. One annoyingly told me "Doctor hasn't done it in 5 years!"
Unfortunately at the time of the examination I was not aware of the form, and  we had other issues to deal with, so it looked complete to me.  Now I don't know, as I cannot quite figure the whole DTP/DTaP/DT mess. We do not have AOS scheduled yet, status is ready for interview.

DTaP is the first dose against tetanus, diphtheria and pertussis -just for children-. TDaP/ TD is the booster, which should ideally be administered every 10 years, from 11 to 64 years old. That is the missing shot on your vaccination records.

You could either get the shot anywhere -CVS, Walgreens- and get a physician to transcribe that in your records now -which would be cheaper than going through the whole medical again- or wait for the interview and receive an RFE

@Shiran On a quick search I found this place; it has just good reviews.

Maybe you’ve already checked on it?

Thanks, I do not recall that sight, I was calling people as they are listed on USCIS page. Primarily just wanted a confirmation if something is missing. Most had very hard time grasping concept of "We do not not full medical, but need to check vaccination"

I will call the place on Monday. Really appreciate you taking time to look!

1. Your DS3025 is not marked as complete.

2. Your TDaP or any of the variations have not been administered within the last 10 years which is the requirement.

3. I can't see the far right column to see any of the codes. So I cant see if you were given a waiver for flu or a waiver for anything else for that matter.

IMO you will need a new I693 vaccination supplement.

I'd still wait to do anything until after the interview though.

There are no codes. So far we didn't get any courtesy letters about vaccinations/I-693. Still waiting for interview date.

Then it's really filled out incorrectly. My recommendation to all of you all who are being screwed by your Panel Physicians is to report them to USCIS.

A courtesy letter wouldnt mean much anyways. It's a randomly issued letter. They would not even have accessed your medical exam in your A-file prior to issuing you one. Think of it as junk mail.

My advice still stands. Yes you will likely need a new I693 vaccination supplement but dont bother until after you are interviewed. You never know....your IO could be just as clueless as your Panel Physician and may accept your extremely incorrect DS3025. Then you would have wasted hundreds and hundreds of dollars.

@Shiran

We are in the same boat. We have few remaining vaccinations. Employee clinic will give all necessary vaccinations and fill out the form(s) but they are not USCIS authorized location. So I am thinking we just get vaccinated, file for AOS and wait to hear back.

Any suggestions?

I found a local USCIS person who said he will sign I-693 if we bring evidence of vaccination for about $250. So far, we do not even have interview date. Will make decision at that time. The guy that I found actually reviewed DS-3025 by e-mail and responded that the only thing is missing is Tdap due to 10 year requirement. If you want I can PM you his e-mail, and maybe he will be able to review your DS-3025. Paying $250 to avoid a possible hassle of dealing with RFE after the interview might be a reasonable trade off.

That would be great. Thank you. I know we have few pending vaccinations. We should have taken them back in Almaty. I wasn't expecting such requirements.

I am basically trying to figure out if the civil surgeon would understand that she doesnt need the 693, and just needs the vaccinations, or if i can just get her the vaccinations and take to the civil surgeon and have them update the ds-3025? She will be covered with my insurance as of Sept 1, and fortunately there is a place that has civil surgeons and also does the vaccinations, but i havent been able to get in touch with anyone to verify.

Our sheet looks similar to Shiran where there is nothing listed on the most right hand column, from the same person

They all "understand" but it's about the money and charging you to redo everything because there is no policy that says they cant.

..if i can just get her the vaccinations and take to the civil surgeon

Yes you can do that

...and have them update the ds-3025?

They wont update the DS3025. That form is for Panel Physicians abroad only. The US version is the I693. It's up to the CS whether they will only complete the vaccination section of the I693 or if they will make you redo the whole thing. Again, its a money issue.
