G-325A married name or maiden name

Hello everyone.

I am filling out my G-325A, my question is, what i need to put for my family name?is it my married name already?or they want my maiden name?

Whatever is your current legal name. If you've changed your name to your married name, that's the name to put on the form and then you put your maiden name in the specific section for other names.

In the form it says there Applicant's last address outside the united states of more than 1 yr. And im living in Philippines,and since birth i am living outside the USA. What should i put to this 0ne? Should i just put n/a? Or what should i do?

I am the benificiary.
