My wife and I had to complete the civil surgeon part of the Aos process. We live four hours from Chicago and Two from St. Louis so we chose to go to St. Louis since it was closer we Chose a Filipina doctor since my wife is Filipino. She quoted us a price on the phone of 100 to get this done so we got there and I paid them 100 at the cashier Then she said we needed some vaccinations so i paid 70 for them. Then after that she said we need to redo the exam even though the civil surgeon instructions say if the exam we had has been within a year you do not have too. She said she couldn't finish the transcription because she wasn't the one that did the original exam. She told asked my wife to see the instruction forms which led me to believe she didn't know how to do her job. I took the day off work drove two hours there waited there for five hours then drove two hours back home. This woman told me that we had to come back on wednesday to do the rest. The guy at the desk said in order for us to put this in a sealed envelope would require another 100 dollars. So basically this woman told me it would cost 100 and so far it has cost me 170 plus gas and time from work. If we go back on wednesday it is gonna cost us another 100 just to see the doctor again plus another 100 for the sealed transcription and maybe even another 300 for another medical exam. I would like to file a complaint against this Civil Surgeon because they are either INCOMPETENT or trying to SCAM me either way I knew more about the transcription than they did. Anyone have a number to file a complaint against this scammer/incompetent civil surgeon.

Scammed. The only parasites in this process bigger than attorneys are Civil Surgeons. They can require anything they want in return for their signature. What is "required" does not matter, if you want their signature, you pay what they demand.

Alla and I have worked with our Senator Leahy here to introduce legislation allowing ANY licensed US doctor to sign off on the I-693 for immigration purposes The original legislation was not passed but he may add it as an amendment to something else.

Definitely you're getting scammed. We got my wife's booster vaccines for free because I added her to my insurance and its preventive care. To transcribe and seal cost us $75 at a civil surgeon. He took the time out to have us fax in to review her current vaccines before our appointment, and tell us which she needed and to try to get them through my insurance because it would be cheaper or free. He also said if that doesn't work, he knows of services that can give them cheaper than he can, although he would do if we preferred it that way. From my point of view he went above and beyond what he needed to do!

Definitely being ripped off. You did not need the exam, and I have never heard of a CS charging to put the I-693 in a sealed envelope for you, much less $100 for that. :bonk: Bring an envelope and tell them you refuse to pay $100 for a simple envelope and will even lick it for them to make it sticky for free.

I do not know who you report bad civil surgeons to, but I hope you do file a complaint somewhere with someone. It is not right that the USCIS lets them get away with doing this kind of thing to people going through a stressful and already expensive immigration ordeal.

If I were in charge of All Things, I'd make it clear to all Civil Surgeons that the I-693 is to be transcribed promptly and completely for $50 maximum, and that any attempt to require an unnecessary medical examination would result in the CS designation being stripped and Federal felony charges being filed.


06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

Mine was $115 for the I-693 handling, they didn't do the vaccines but I went to my own doctor for that and got it paid for by my insurance.

When we went to the CS he listened to my wifes' heart, checked her pulse, had the nurse pull some blood for a Syphillis test and Transcribed the Immunization records, then sealed an envelope and called it a Physical. all for $115. This was a ripoff that I filed with my insurance company, coded it myself, and was reimbursed. There was no fraud on my part I told the insurance adjusters/coding department all of the details and they helped me get re-imbursed.

Thanks for all your replies! We went back to the CS on Wednesday April 18th. We were running late for the appointment my husband wouldn't accompany me because he was so angry and he said he already knew what was gonna happen he'd get arrested coz he'd yell at the staff for insisting I do the medical exam. So my mother-in-law accompanied me instead. Thank God I DID NOT redo my medical exam. All expenses were $70 for the vaccine shots and tb skin test and $100 for transcription and blood work. So what happened was a big misunderstanding. They did not know and understand the process. Just gotta insist on not redoing the exam otherwise they would really rip you off.

But a TB test and blood work is part of an exam, and those were not necessary either. All you needed was the missing vaccinations and vaccination transcription onto the I-693. I am just glad you are done with that part and can move on with your AOS. Good luck on the rest of your immigration journey! :)

I talked with my CS about this. and the TB skin test. I said she just had her exam last September in St. Lukes. All she needs is the vaccine records transcribed. The CS replied, he had written the USCIS about it. they replied with ALL people have to have the TB skin test. Even IF they have the BCG shot in which they will read positive for TB. At least we have the x-ray from St. Luke's which proves no TB. It is very important to have complete vaccinations records, get a medical check-up by your local family doctor after getting here. (have all the tests and vaccines done by the local doctor.) then go to the CS with all records in hand needed for the form. This saved me much much much time in the office. We have to get tetanus and the TB skin test which will cost me additional $75 but I plan on submitting all to my insurance. the appointment was $160, but that also covers the additional two appointments for the shots and TB skin test reading. I don't like it, but at least the form will be done.

I agree any licensed Doctor should be allowed to fill out the form.

it should be amended to "A Physician who is in good standing with medicare/medicaid. The Family Care Physician must not have had issues with medicare/medicaid for the last 5 years, nor have been rejected by medicare/medicaid in ANY state or have license revoked in ANY state or ANY country. The Physician must be recognized as a PCP provider by medicare/medicaid. Urgent Care Clinics, Hospitals, Laboratories, Ambulatory services, Home Care providers, RN, LPN, DO, and Physicians who are a specialist in a field as recognized by medicare/medicaid are exempted from being allowed as recognized Physicians. Examples of specialists could be, but are not limited to: Podiatrists, Neurosurgeons, OB/gyn, Internal Medicine"

The CS lied to you. The TB skin test is part of the medical. K1's do not require a medical. How do i know this? (Aside from reading the instructions) I REFUSED to have a TB skin test done (or medical). My CS still signed the transcription (as i instructed) and told me if i got an RFE not to come back to them. Surprise surprise I was approved without RFE or interview.

A medical check-up here upon arrival is not required for AOS. Sounds like you were scammed as well.

Us too,we found a CS to transcribe for 26 dollars. Note that was after calling about a dozen different CS, some wanted to do a complete physical with a price tag of 1100 bucks. Big scammers

Exactly. A K-1 that is adjusting status does not need another TB test. That is part of the medical, and if you had your medical within the last year, another TB test is not needed. The TB test is required for those that are required to get the full medical when applying for AOS. This does not apply to a K-1 who recently underwent the medical in their own country.

They do an x-ray at the overseas medical because it is a better indicator of whether or not a person has TB than the skin test, since the skin test can give off false positives. If the x-ray from your medical showed you did not have TB, then why would you need the lesser reliable skin test? My husband had no extra blood work or TB test, just the vaccination transcription(for $25) and was also approved without an RFE.

WE had no TB skin tst at St. Lukes. Probably because of her having BCG and X-ray in April which showed her being clear. Not everyone is given the TB test at St. Lukes. I called around to several CS in Phoenix. All wanted the same thing. Even though she had a physical in September, I just had to give up and go to get it done. Am I frustrated, yes. But there are about a dozen CS in Phoenix, with about 8 practices. (some are part of the same practice.) The k-1 visa process is not well understood. Most are IR-1/CR, or H1b visas, So CS tend to go off the deep end. Debating with a CS on the forms just gets you escorted out the door or hung up on. The best thing I have going for me, is I can submit a claim to my insurance company and get reimbursed or applied to my deductible. Yes, I feel I was scammed. but with not many CS to go to, what else can I do? Travel to Vegas? LA, SF? SD? makes no sense when I have CS right here in Phoenix with the local USCIS office/lockbox in Phoenix as well. Not worth the questions at the interview. If we get asked at the interview why a full medical I will tell them in no uncertain terms why. but otherwise, not much else I can do apart from help reform the system.
