US Immigration from Philippines

Hello! Filipino beneficiary here.

I just received my CENOMAR by mail today, which I had ordered from PSA online. It s for my old name at birth. When I compared it to my CENOMAR from PSA I had attended an appointment for (for my current name), I noticed my CENOMAR from online does not have a barcode.

I am aware barcodes are important on documents like the appointment letter and the DS-160 confirmation letter. Is it also important to have it on CENOMARs that I should buy another, or not so much? Thank you!

For the I-751 form (Part 5: Information about your children) do I have to include our child who is born here in the US?

I also have a child from previous marriage that immigrated here with me.

I am not sure if I need to include my child who is a US Citizen on the ROC form.

For Ir-1, Ir-2, does anyone now know which covid vaccines are required to get through Slecs for medicals? Wife has had 1st 2 Sinovac vaccines over a year ago. Also, 14 yo has only had 1st vaccine and no more have been available for kids since his 1st about 6 months ago or so. Was told before that if not available, could possibly get a waiver is what I think I read for USCIS.

Also, if not mandatory, we would prefer not to have any boosters for wife, if not required.

Hoping someone has up-to-date inform on the current details.

and i heard that we need to make a letter of intent?(not sure what the correct legal term)
what's the correct format? i looked around the forum and mostly its all about the letter of intent to marry(k1)
does it also needs to be notarized?

Hello, Filipino beneficiary here looking for others who had experience getting 2 CENOMARs with different names! I have a PSA appointment soon.

I need another CENOMAR with my birth name. My parents were not yet married at that time and so my surname was different before.
I want to get a CENOMAR on that exact old name, but there seems to be very limited resources on what it was like for those who got them successfully.

Did you guys need to provide a specific document? What should I bring to PSA for my appointment to get that CENOMAR with my old name? Is it fairly simple and smooth process like with getting CENOMAR for current name? Please help!
