It doesn’t sound like a outright refusal but more like AP.  Try posting this in the Canada forum for more consulate specific answers.

"Refusal" seems to mean that the consulate cannot make a final decision at this point and they have put the file in a queue for further work.  Now this may mean they have requested more information from the applicant, they need to do further background checks, or the applicants passport is simply waiting in line to get the visa applied.  Refusal is not the same as being denied.  Consulates used to use the catch-all phrase "Administrative Processing" (AP) for this, but now AP seems to mean that someone is actually working on an application rather than the application just sitting and waiting for something to happen.  I have heard of people reporting cases showing Refused, then moving to AP, then back to Refused several times.

 

As to how long this may take, no one can really say as it is based on the workload involved, or what information the consulate may be waiting for.  Just be patient, and if this current refusal is based on the consulate requesting information from you, respond as soon as you can with the information requested.

OP, Supplement Questions does mean you will be asked to fill out DS5535. Your case has been selected for extreme vetting, unfortunately. It will now go through additional background checks. Your visa isn't refused, just temporarily on hold until all this work is completed. There is no timeline, it can take anywhere from 2 weeks go 2 years. This number is based off of other cases here on Visajourney and Facebook. Good news is that one person in the thread linked (also at Montreal consulate) was able to get his approved visa within 45 days. We are hoping the rest of us have similar results. If you'd like, join this thread. We all share our updates there. 

 

Edit: forgot to add reasons for denials. No one really knows but people who were born/lived in or visited certain countries are more likely to be flagged. These countries are often Muslim/African countries. There are also name checks that happen, perhaps you have the same name as some bad person out there and they need to confirm you are not them. Your educational background could also be a reason (google technology alert list). This can also happen if you've worked/studied in certain countries (I believe that list is India, Pakistan, Israel, China and Russia).
