OP, in direct response to your question, unless things changed recently, USCIS is not allowed to ask the USC about any benefits she receives, nor can they use it against her or you.

But if I am not mistaken medicaid is means tested, so it doesn’t matter if they “know” so much as the issue of her low income, which they will know.

That worksheet - along with several of the factors noted on it - is new with the updated public charge rule that was implemented on Feb. 24th. I'd have to go through archive.org to verify if that particular factor was there or not before...honestly, my memory today is a bit mush so I don't want to misspeak (hence why I made sure to provide the source ) .

Based of this worksheet I can see that in my case I have about 13 or 14 positives and 3 negatives which includes the sponsor receiving public benefits! Can I consider my case to be strong?

No, it means that if the petitioner (primary sponsor) is financially unable to sponsor the immigrant, an eligible joint sponsor may be used.  "Lining up" just refers to having it arranged ahead of time.
