Since you are paper filing I would also add an excel spreadsheet print out (showing date left and returned and number of days out and where etc -  title the sheet number of days out - include the with the total number of days calculated for them) - make it easier for them to review it

 

but that’s me - I probably have too much experience with government regulators lol 

Oh yes! With government its always like that, yes I am putting dates from to and how many days each trip

I didn't include a cover letter. You'll be fine. Just make sure your PDF files you uploaded have descriptions on them.

I didn't....unless your circumstances are different where you need to submit something like that? I disclosed my speeding ticket because I was not arrested, just given a traffic citation.

If the instructions says you need to because you answered yes on any of the questions regarding arrests etc then you need to. If you have never gotten in trouble with the law then you don't.
