There is no limit to the number of visas for spouse of US citizens.

Sorry for saying sister when it's your cousin. As a US citizen, there is no numerical limit for immediate relative. Did she submit proof of US Citizenship with the I-130 application?

I'll be honest, maybe your sister and your cousin should find someone new to help them with their cases. Either a visa agency or lawyer.

If you're filing for them and something is wrong in both cases and you can't explain why to the point where we can help you, perhaps they need someone else to help them.

If the app made it clear that she is a US citizen (submit her naturalisation cert etc), then the only thing I can think of is that you/she missunderstood; even with NO numerical limitations, it takes about a year for the process, from filing to visa interview.
