Hi, our daughter is 5 months old. We can add her to our DS 260 file now. Should we (the parents) complete her part of the application as well? Or are children this age exempt from completing the form? She doesn't have a passport yet, just a birth certificate. The form asks for information about the passport. Should we go ahead and get her a passport asap or send in the form as it is and get her a passport closer to our interview date or before departure to the US? Thanks!

As Emily says, you need to fill in a DS260 for her. And get her a passport. You don't need it to fill in the form - you can update it later with the details when you get it.
