I went through Vancouver and it was about 30 minutes for customs including the part in secondary. I allowed 2 hours for it and don't regret that at all even though I didn't need it because I wasn't stressed at any point.
 
I just brought mine. Handed the guy both and he handed the old one back and put a post it note saying "scan" on my new one. Same thing with the updated i864. Didn't delay me at all.

It really depends case by case. I can attest to that. Good thing is that its electronic so the embassy gets the medical pretty quickly.

I finally got my tracking number for my passport being sent back to me today! 
 
My interview was a week ago (Monday, November 9th), and the status didn't switch from "Ready" to "Issued" until Friday (Nov 13th), and then they finally created my tracking number today. 

Finally a tracking number! This has been a testing process. Pretty sure the embassy lost my wife's original fingerprints. They asked her to fly back to Montreal literally for 15 minutes to redo her right hand.

So the embassy called my wife and said her fingerprints did not register correctly so she has to go back to Montreal. Ridiculous. They make mistakes and we have to pay for flights and travel. I have a feeling that they are going to find other issues after the fact and take even longer. Why can't they be up front from the beginning you literally can only ask questions when they feel like answering them.

They said come to Montreal. They were vague as usual. They told her not to book her ticket until tomorrow when they call back to confirm a day she can come to the consulate. My wife also has an American sim card because she had to do a lot of her medical exams/ interviews in the USA and the agent was asking why her cell number is a USA number. It's just a sim card because it's cheaper than having to pay international plans. Now i think they have gotten suspicious.

Still no approval guys. i can't think of why. My wife said the interview was fine. Medical was the only thing on the 221g form. I am trying to think what could be the hold up. It seems odd that it's taking this long when people who interviewed after us with 221g's have had their visa approved. We have never broken any rules at all. Not even speeding tickets.

My interview at 9:45 went well. The lady said as soon as my new medical and new police certificate is sent to them they will send my passport with the visa. Now just waiting for those two documents ❤️

Same. Our medical has been there for over 2 weeks now. Still saying that the officer needs to review the case.

My wife's case got expedited because she is in the medical field but they gave her such a short window to interview. They even told her that because her case was expedited even if she tried to re-schedule her interview it would be  way later. She had to do everything in a very short time so her medical was late. Now here we are. Medical has been sitting at the consulate for 2 weeks and nothing. She has to to complete a board exam soon in the US or her career will be bust. They have her passport so she cannot do a thing. So the expedite really didn't do anything. They give you hope but take it away anyway.

Finally received an email reply. They confirmed that the medical is there. They said they are waiting on the officer to "promptly" review the case and approve.
 

Got an email confirming they did receive our medical that was sent on Friday-- so hoping they will hurry up and finish the review & issue the visa! Our goal was to meet the day after Canadian Thanksgiving and drive down. I don't really think that is going to happen. but maybe some time next week. 🤞
