What is the physical presence requirement in usa for CBRA .. 5 yrs including holidays out of usa ??

5 years physical presence in the US, two of which must be after age 14. Holidays outside the US are not physical presence in the US.

Difficult to tell, depends how long those holidays were.

Sounds close, you need to check your dates.

You’ll need to work out how many days you actually spent inside the US.

From 2016 I went out for 1 to 2 months in a year

In CRBA process will they ask previous  passports in required documents

You will need to be able to show proof of when you were physically present in the US, a passport would be one way but not the only way to do that.

Yes but I filed tax return every year in USA I have 5 yrs tax return transcripts

That is another requirement but not relevant to CRBA.
