So, I have my case finally ready to be scheduled (as of last night).

However, I never sent in a medical. I never even received a i693 deficiency notice.

Do I still need a medical? Is it waived? Will the letter come eventually? Has anyone been in this situation?

You did a medical for the visa, why do you need another?

YMMV

Chances are if you have not done a medical, it would be brought up in the interview. It can be submitted during the interview, based on the filing instructions:

You (not the civil surgeon) must submit your Form I-693 to USCIS. If you are applying for adjustment of status, you may submit Form I-693:

  • By mail with your Form I-485, Application to Register for Permanent Residence or Adjust Status, to the location specified for your Form I-485 (see Direct Filing Addresses for Form I-485).
  • By mail after filing your Form I-485 to the location specified in your most recent communication with USCIS (for example, a Request for Evidence letter from USCIS).
  • In person at an interview in a USCIS field office (if an interview is required).

so you filed AOS and you got an interview.  You can take the medical to the interview.

It is never waived

Yes you still need medical if you filed AOS and did not send I-693 with your application

I think it's best to have it. Especially if the officer look for it during the interview. In my case, I needed to do another one, my first one expired got a i693 deficiency notice. I brought it to the Interview, but the officer didn't look for it. But don't take chances.

My i-693 deficiency notice came about a week or so after I was moved to "ready to be scheduled for an interview".  It will come.  You will need it so you may as well start getting it done now.  Also, "ready to be scheduled" is very different to "scheduled".... it may be a month, it may be 18.
