Assuming you married within the 90 day period , the most common reason for getting a denial on filing is because a required form was not submitted with the application. But you will need to wait until you get the letter to know the reason why. Then you can address the issue and refine

You did not get an RFE ? straight up Denied

If a form is missing ..such as an I 864... the application is now being denied straight away. In the past an RFE was issued , but that’s changed over the last 12 months.

She is K1 visa and all out of status is waived on being married to a citizen.

from my understanding if a mandatory document was mission the petition would never be accepted.  in OP's case the petiton was accepted so this adds to the puzzle

Most likely causes are either a required document being missing OR failure to respond to an RFE.

Did you move after filing the I-485?

I've seen applications accepted with missing required documents. For example, a missing marriage certificate, no I-864, etc.

They sometimes RFE for it. Sometimes they don't RFE and outright deny.

No we didn't move. We never got an RFE. The last update we received from the USCIS was that they got my finger prints and that was back in August.
