I’ve read to change your visa to the official green card, you need to do an medical exam again and another interview. I’ve already done the medical for the visa k1, will I have to do it again?

Probably not.  Sounds like you would benefit from reading the AOS guides.

You get to do the medical again if it is over 1 year old when you file the AOS.   This shouldn't occur as the time for entry into the US is six months, then 90 days to get married.

Do you even need to file form I-693 for K1 visa holders? Or just file it out and send it in with your previous medical reports?

If your medical from the K-1 visa has lapsed over a year, then you're required to do it again.
