Is a waiver required when filing a new k1 for the same beneficiary within less than two years of the prior k1 petition being approved and visa denied? Where do I send the waiver?

Thank you!!

Ahh okay. So I only need to check one of the boxes and attach an explanation?

I wasn't sure if I had to do anything in addition to checking the box. :)

And as a part of the explanation you include that you are requesting a waiver from the multiple filings limitations

YMMV

One other question.. I select the first box even though I was convicted of a misdemeanor in 1998?

I really feel that I did not front load the packet enough as I thought we would show the bulk of evidence during the interview.

Also, I was not able to attend the interview which is heavily encouraged in the DR.

What are you waiving?

Because filing twice for the same person does not require a waiver.

What exactly did the denial slip say and why was the case denied?

It does for an I-129F if fort a fiance/fiancee if the previous petition was filed within the past 2 years (not exactly the same set of circumstances described by the OP, but presumably still fits):

https://www.uscis.gov/sites/default/files/files/form/i-129finstr.pdf

Quote

Item Numbers 5.a. - 5.d. Multiple Filer Waiver Request. You must request a waiver of the International Marriage Broker Regulation Act (IMBRA) filing limitations if:

1. You are filing this petition on behalf of your fiancé(e) and you previously filed Form I-129Fs on behalf of two or more fiancé(e) beneficiaries; or

2. You are filing this petition on behalf of your fiancé(e), you have previously had a Form I-129F approved, and less than two years have passed since the filing date of your previously approved petition.
