If a couple is married while one is in the US on a visitor visa, after the wedding can she return home? No papers have been filed but there is a marriage certificate.

Yeap, totally can return home. Thereafter the US spouse can petition the foreign spouse for a spousal visa. However, the new foreign spouse can actually adjust status within the US without leaving.

Begs the question why you think it would be an issue leaving?

Personally I think it is because there are so many questions like this.  Of course a spouse can leave the US, the issue is always getting back in, but many interpret that to mean the US Govt. would hinder those leaving.

Yes, she can return home and must return home because she is on a visitors visa, She doesn't have the appropriate visa for staying in the US. Unless she applies for AO,  she can stay.
