My wife got her certificate in Kanagawa and these are their stated requirements. The requirements may vary depending on your wife's prefecture.

  1. Valid passport (its amendments such as name, etc., if any, must be completed prior to application.)
  2. A Certificate of Residence issued within the last six months, or a Japanese driver’s license.
  3. A document issued by the government, or other public agency, representing the country (or territory, etc.) you are submitting the certificate to, showing your name. She used our NOA2 approval letter from USCIS.

You can follow the medical exam instructions here. Keep in mind that the results are only good for 6 months and must be valid when your wife enters the US with her visa. These are the general required items to bring to the exam, but each practice may have their own specific list or additional forms.

  • Your visa interview letter. We waited until we had our interview appointment scheduled, but the office told us we could have scheduled just using our NVC case ID. I would recommend waiting to schedule at least until you have all required documents to submit to the NVC including the DS-260 to minimize risk due to processing delays. The safest bet is of course to schedule as soon as you are given an interview date.
  • Your passport
  • Four (4) recently taken passport-sized color photographs
  • A copy of your immunization records.

Some guides out there recommend getting your immunizations in advance at a local clinic before going to the exam at an approved clinic in order to potentially save some money. DO NOT DO THIS. Your average clinic in Japan does not have a license to carry the exact immunizations required by the US, specifically MMR. We made this mistake and it caused us no small amount of stress to get another appointment scheduled to get the right immunizations as well as waiting the requisite amount of time after the previous administrations. It's not worth the ~2000 JPY we may have saved otherwise.

 

That aside, my wife went to the Kobe clinic and had no other problems there.

Is it okay to take the antibody/titer test at a local hospital where we live? My wife, being from Vietnam, doesn't have any vaccination records, but she does recall getting some vaccines  when she was younger. Another thing I'm wondering is if she would need some vaccinations, would she need to make multiple visits to the approved physicians in Tokyo. From where we live in Japan, flying is often the cheapest way to get to Tokyo, but it can get pricey.

I made both my MMR vaccine and my titer tests in my local clinic. Took all the receipts and tests to the uscis approved clinic. Had no problem at all. It wasn’t to save money but 1) I already had one of the titer tests from obgyn as it is free to do. 2) it takes up to 10 days extra to wait for a titer test.

Just take all the tests/receipts/records with you to the clinic. I took my titer tests with no problem and they just put the necessary information during the medical. You can always call and ask the hospital you want to use for your medical if it’s ok. I did and they told me to just bring the papers.

Do I need to get this? Is this requirement only applicable for Japanese? I am a Gaijin.
