1. Yes, include the EAD (employment form). It is free when you submit it with your AOS/I-485, which costs $1070. You should also include an application for Advanced Parole (AP), which would allow you to return to the USA if you had to leave before receiving your GC. That is also free.

It's not a computer glitch. They just call it that because they don't want to admit they got hacked. No visas are being issued at the moment. They also had issues issuing passports but that's up and running again. They are currently prioritizing humanitarian visas and adoption visas. They do now know when they'll start issuing visas again so people who have been approved will be waiting for a while. People with visa appointments have been asked to re-schedule.
