Vaccination Supplement???

Hello All,

I am in the US now on a K-1 Visa and have married my husband-Woohoo! Now, while filling out my i-485 it says I need to attach a vaccination supplement, what is that? Where do I get it?

How long do I have to apply for the AOS, my Visa expires on Nov 25th,2008, but as I said, we're already married, does that make a difference?

The vaccination supplement is form I-693, and section 5 needs to be completed. You must take your current vaccination record (from your home country) and take it to a US CIVIL SURGEON (you can search www.uscis.gov for a civil surgeon in your area), they are the only ones who can complete this form. They will transcribe your vaccination record to this form. If you don't have a vaccination record then they must give you the vaccinations needed or test you for imunity. The requried vaccinations are MMR, Varicella and Td. You can apply for AOS now, as long as you have all the requried documents.

DS3025 cannot be enough. I was told to send I-693, too. This is the form that you need to attach. That needs to be completed by a USCIS doctor. If you met all the required vaccinations, you can simply find a doctor through USCIS website. Some people could get away with only DS3025. But, you are more likely to get RFE.

When I did POE, the IO was really strict about this medical form. He reminded me to complete with civil surgeon. And it depends on the age. From 2007, they added some new vaccines. I had to take TDAP and HPV.

Yes, I included the vaccination worksheet and had no problems. But that was 4 years ago.

There is a very long discussion about this topic at the top of the AOS thread in a sticky. The more recent posts would give you the latest experiences. We personally sent in just the vaccination supplement DS3025 just to give it a whirl. We'll get Dr. Arnold to transcribe the immunizations if we get an RFE. (See the thread to find her number) Also there's lots of info on the new TB skin test requirement in that thread, which will probably get us an RFE too. We just decided to try it with what we had, and deal with RFEs later if needed.

To have your records transcribed to form I-693 with the CIVIL SURGEON varies around $20-$50. If you need to take vaccinations, that's a different story. I'm not sure how much the vaccines are.

Some civil surgeons may only charge $20-50, but those are the minority. The majority of them are cold-blooded crooks. They will try to convince and scare you into having a full medical, even though they know that you don't need one. Even the instructions say that they don't need to do one, and they will come up with ridiculous excuses/reason why you have to have a full exam again. All they need to is copy your DS-3025 onto an I-693 (vaccinations part and sign part 5) which take about 12 minutes. But they want to do a full physical/bloodwork and charge $200-400.

Do not let the doctor try to get you to do more than you need, a new medical is not needed if you've had your medical in the last year. It says so clearly in the instructions for the I-693. David and I paid $25 for the transcription. Call around to different Civil Surgeons or use Dr. Arnold on the first page of the I-693 info (see link below).

Well, in our case we are exhausted and just wanted to send in something to get in the pipeline rather than chasing down a civil surgeon 60 miles away. In 6 wks, I've evacuated my home for a major hurricane, had my town virtually shut down for lack of electricity for 2 weeks, flew to the UK because my fiance had an accident and broke his collarbone and ribs days before the shippers were picking up his household goods, flew back to US with fiance and put together a lovely wedding in 4 days, filed insurance claims for hurricane damages, and are still trying to get roofers and general contractors to have time for us while rain falls down inside the wall in our bedroom and black mold is growing through the walls. Oh yeah, and we got AOS/EAS/AP papers filed somehow. We are together and we are happy and just don't care if we're delayed another few weeks by USCIS. By the time they get around to looking at our case, maybe we will have done the I-693, which I totally agree that we will need. I just feel good that we have a NOA1 and are in the October box.
