Hello!

I'm kind of confused about the vaccination supplement that is needed. My fiance had his physical about a month and a half ago and we'll be filing for the AOS before a year so I think all he'll need to submit for the medical part is the vaccination supplement, right?

His physcial was done in London, but, after his physical, he didn't get any kind of documentation. Before he comes to America, should he get something from the doctor that did his physical? Does he need to get a vaccination record from him? What should he get before he leaves?

If he can get the vaccination record from his medical examination it's ok, but if it isn't possible he can get his vaccination here in USA with a community hospital and the records are valid when he will need his I-693a for AOS.

OK, I also need some clarification on this...In the AOS guide, it says "I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.) "

So we take the 3025 that my wife got at her medical exam (it was signed and dated by a doctor), and then take that with the I-693A to a civil surgeon, who then fills out the I-693A? Or do they just fill out the vaccination supplement? I guess part of why I am confused is that they kept the medical results when Ruth entered the country. Does that have anything to do with this at all? To re-state it, we need to look up a civil surgeon and have him/her fill out only the vaccination supplement? Or the entire I-693A? She had her medical at the embassy in Juarez but didn't get any vaccinations then...Thanks tons. I think this is the only area that I am not clear on for the AOS package (yeah, right!)

Many people (my wife included) submitted only the DS-3025 as proof of vaccinations (the supplement) with the I-485 application and were approved without incident.

Cool! Thanks for the quick reply...I was worried there for a minute....It's not real clear in the instructions, in my humble opinion...

But also be prepared just in case you do need the supplement done. You will need a copy of either the DS 3025 worksheet or vaccination card. If your going through Atlanta they have given out a few RFE's or asked for it at the interview, for the supplement even though all vaccs are up to date (me and 2 others on here that I have read about with that office so far). It just depends on who is looking at your case.

Ask for second copy of the doctor's report in a sealed envelope... just in case the USCIS loses your medical report, as can often happen. Then if you get an RFE for the medical you won't have to re-do the paper work here- which would mean re-doing the exam and tests which is $$$.

Also, the USCIS requires a TB skin test- a chest xray or other test is not valid. In many countries they don't have the skin test and you have to take it in the US, but I bet in England they have the skin test?

tell him to get the ds3025 from the doctor, normally they give you a copy after the medica, thats what theyre supposed to do at least!

then, before you guys file for aos, call dr. l. e. arnold in austin tx 512-444-3131 she will transfer the ds3025 to the I693a for $20. that way you dont have to find a registered civil surgeon near you that wont rip you off and youre definately safe regarding the aos application!

some people were fine just submitting the ds3025 some got a rfe for it and then had to get the I693a anyways.....

@fedekat i had a xray taken for tb at my medical exam and im already a green card holder. a lot of countries dont over the skin test anymore, so the only thing you can get is a xray....

I agree as the x-ray is a much more definitive "test" than the mantoux, which btw produces a significant amount of false positives.

I received an RFE for not having a TB skin test.

From the USCIS rule 8 CFR, section 103.2(a) "applicants 2 years old or older will be required to have a tuberculin skin rest."

Lucky to you for getting a green card without one, but perhaps the rule has changed or was not enforced in your case.

The I-693a IS the vaccination supplement form. Take the DS-3025 to the cs and they will transfer the data to the form.
