AOS interview....confuse about form I-693????help please!

Blessed day all,

So filed my AOS, done biometrics (Honolulu) , today received the appointment letter for my initial interview.

It states" bring the following items".............

2. A completed medical examination (form I-693) and vaccination supplement in a sealed

Evelope ( unless already submitted).

That part confuses us.

So my question is, Do I need to see a surgeon for that? Or do I need to just bring my medical records from overseas and no problems during my interview?

I have all my medical records from Saint Luke's clinic. In fact

When we submitted the AOS package, we included a copy of DS-3025.It's less than a year since I had my medical exam at Saint Luke's clinic.

Please help if it's necessary to visit a surgeon or no. Thank you so much.

If you did your medical overseas and have already submitted it during the AOS, no need bother they will have it on the day of your interview and it will still be valid provided it's less than a year old.

Your medical exam within a year is great but are your vaccination up to date as well? If they arent, you will need to visit a civil surgeon to receive them or have them transcribed onto your form if they arent accurate/up to date.

I received my Tdap, MMR and varicella vaccine on January 28,2016.

My schedule for AOS interview will be on October 11,2016. So you think the copy of my vaccination record (DS-3025) will be enough?

How does this match up to your documentation?

• MMR (two are required but if you have "insufficient time interval" Code B in the last column, then you get a waiver)
• Td or Tdap or DT or DTP or DtaP (One date must be no longer than 10 years ago)
• Varicella (Either a check by "Vaccine" with a date, OR a check by "Varicella History" if you had chickenpox. History gets you a waiver)

If so I would say yes..

That's how my vaccination sheet shows.

Looks like you need a second MMR. If you dont have a second I say you need a visit to the Civil surgeon for the vaccine and update your documents.

Only one dose in a series is required. If you had at least one dose marked off on the DS-3025, then it is not a requirement to get more doses in order to AOS. Read all of the pinned thread, post #1 and #2.

Q. Certain vaccine series can only be completed with multiple visits to the civil surgeon. Am I required to complete the entire series before the civil surgeon can sign the Form I-693?

A. You are only required to receive a single dose of each vaccine when you visit the civil surgeon. You are encouraged to follow up with your private health care provider to complete the series. Once you have received the single dose appropriate at the time, the civil surgeon can sign and certify the Form I-693.
