I-693 or DS-3025

I see conflicting reports regarding what is needed for A.O.S. I have a photocopy of my DS-3025 that I had done earlier this year in London, do I still need to fill out a I-693 as well and have it signed by a civil surgeon, or do I need to fill out just my details on the I-639 and send it with a photocopy of my DS-3025.

no conflict... DS 3025 is accepted if it was complete at time of medical AND was included with the DS 2053 in your brown envelope at entry.

As per the I-485 instructions:

8. Fiance(e)s.

If you are a K-1 Fiance(e) or K-2 dependent who had a medical examination in the past year as required for the nonimmigrant fiance(e) visa, you only need submit a vaccination supplement, not the entire medical report. You may include the Vaccination supplement with your Form I-485.

If everything is complete on your Vaccination worksheet DS-3025 that you got from London then just send a copy of that.

Any ol' DS-3025 may not be acceptable. They'll give you a DS-3025 whether you got the required shots needed for AOS or not. Some doctors fail to sign them. Some fail to complete the results section. I will say Knightsbridge clinic is very good at ticking the right boxes and signing if you got your shots and told them you had chickenpox.

There are certain things your DS-3025 should have to be considered complete.

Look at your DS-3025. If it has the following items, you don't need an I-693.

1. Shots marked (at least one of a series)

• MMR
  
• Td or Tdap or DT or DTP or DtaP (date no longer than 10 years ago)
  
• Varicella or VH written by it if you had chickenpox
  

2. Everything else marked not age appropriate or medically appropriate.

3. RESULTS section filled in with "incomplete". AND "may be eligible for blanket waiver"

4. Signed and dated by the doctor

If it's not completed like that then see a civil surgeon for the I-693. If it is complete, send a photocopy of your DS-3025.

Do NOT do what UK person ECWilloughby did recently and send in an I-693 without a doctor's part filled in along with the DS-3025. Do one form or the other.

The USCIS is supposed to have your medical files, but sometimes lose them or fail to match them up with your AOS. Do not get another exam unless you get an RFE that says they do not have results of your medical. It means they lost them, but you are probably stuck with paying for a new exam. Your official vaccination results are also part of that file. We send the DS-3025 photocopy more to remind the person opening the mail that we are intending to use that DS3025 to prove our vaccinations, rather than an I-693. The official "sealed" version is in you A-file (brown envelope) which they are supposed to get and match up with your AOS.

The DS3025 has to have all those things, not just some of them. Incomplete is fine if the box about blanket waiver is also ticked.

You can certainly send it... it will not serve as a replacement to the DS-3025 if it was not included in the brown envelope. The USCIS is very particular about the "chain of custody" as it relates to the medical info.

After she passed her k-1 interview, she went to a medical center for vaccination where she was given: (1) a yellow vaccination record book, (2) a DS-3025, and (3) a sealed white envelop addressed to "USCIS (Vaccination Form)" she was told she needs it for AOS in America.

She was given 3 shots (Td, MMR, and Varicella), all 3 had a date under "Vaccine Given by Panel Physician" and a check mark under "Insufficient Time Interval". Others like DTaP were marked "Not Age Appropriate"

What bothers me is "Measles" and "Mumps" were not marked.

In the Results section, "Vaccine History Incomplete" and "Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate(as Indicated Above)" are both checked. The Panel Physician's name is stamped on the form, and there is a signature.

Do you think it's sufficient for us to just send in a copy of her DS-3025 along with the sealed envelop addressed to "USCIS"?

I found a Civil Surgeon who would transcribe an I-693 for $50 if we want peace of mind. Does the CS signs and seals the I-693 in an envelop for AOS? I plan on sending a copy of her vaccination record book too.

Am I correct to assume booster shots are not required for AOS? I hope "Insufficient Time Interval" doesn't mean she needs booster shots before the I-693 can be completed.

She requires 2 varicella shots and has only had one. She needs to have the missing shot and get a vaccination transcription done.

I only normally speak for the UK medical in London because that is the experience I have. However I agree with you that the original DS3025 is in that sealed envelope and should meet the requirements.

The 3 shots she was given are the ones she needs. There are five versions of diptheria/tetanus/pertussis that are accepted (Td or Tdap or DT or DTP or DtaP ). She had Td so okay on that one. MMR is Mumps/Measles/Rubella combo so okay on that. No separate "Measles" and "Mumps" needed because covered by the MMR. Influenza is only required from Oct 1-Mar 31 so okay with that. She does NOT need any boosters or additional varicella if she sends the DS-3025 which is complete as of the date on it. That is the date of record the USCIS will go by.

I personally would not go to a civil surgeon, but that is your choice. If you do go to a civil surgeon, then your DS3025 date is no longer the date of record for USCIS. Do not send it at all because the I-693 will supercede it. So because enough time has passed where it is medically okay to have a booster, then it would be reasonable to expect the civil surgeon to want her to have any boosters that are due. If you go with the DS3025, then she was complete at that time and USCIS will not expect boosters.

Your confirmation of my initial idea of not going to a CS was comforting and reassuring after being shaken by Vanessa&Tony's comment, "She requires 2 varicella shots and has only had one. She needs to have the missing shot and get a vaccination transcription done." Nothing against anyone, because I believe everyone here is sincere in sharing what they know and being helpful.

The CS I spoke to seems reasonable and would do only what is required by USCIS and did not insist on additional medical etc. Even then, as you said, I wouldn't want to go to a CS if it's not necessary.

My fiancee had her medical and vaccination done in July. She'll be here in September, and I'm hoping to send off her AOS application package by the end of October.

Keep in mind that no flu shot will be needed if the July date of the DS3025 is used, even if your application goes in late October. They go by the date on the DS3025 for determining "complete". But if you decide to do a new immunization record with a CS, then that new date will be used by USCIS for determining if she is "complete". If you see the CS after Oct 1 (flu season), then a flu shot will be needed, as well as, the CS may not sign off without additional shots in the MMR series for example. While the second MMR could not be given in July, it could be given after time had passed. So new form with CS may mean new requirements and additional costs.
