There is absolutely no requirement for a foreign marriage certificate be registered in the US....

The I-130 petition is on the horizon, I have a lot to prepare for that and educate myself on. That process is to come. My main concerns are this upcoming embassy meeting. And I believe my questions concerning that have been thoroughly answered. I thank you all for that.

If not in English, yes it needs translation but authentication or notarization not needed. Sounds like you lawyer is after some extra money based on your not familiarizing yourself enough with forms o procedures. Head up to guides above and read until you can recite all steps through naturalization.

She has a letter of acceptance from the Hospital here, as well as a letter from her Hospital there.

If denied a tourist visa for this (highly likely, given that it is not for tourism, and her previous visa denials which are all on her record), she can simply request a delay in this one month observation experience and do it after the CR-1 visa process is approved about a year after you file the I-130 petition (assuming all goes well).  And as others have said, don't jeopardize her CR-1 chances by misrepresenting any information on the tourist visa application.  A more important question for her future opportunities to practice medicine in the US after immigrating will be the courses, rotations, residencies, etc. that she will need to satisfy to become licensed in the state where she will live.  Spend the money on that or visits to see her during the one year CR-1 process instead of wasting it on attorneys who tend to make mistakes, slow down the process, and charge you for things you can easily do yourself.  Good luck!
