use black ink and make sure it is legible

Did you open the form with Adobe Acrobat Reader? In my experience the script on the form doesn't work correctly with other readers.

If you did, make sure you've check all necessary checkboxes; it won't let you type in places that aren't valid based on your other answers (and it's okay to leave non-applicable sections blank when the form won't let you fill them in).
