Birth Certificate Discrepancy

My birth certificate is registered under the surname of my mother. However, I've been using my father's surname in my passport and other documents. Do I need a notarized affidavit saying both versions of the name indicate the same person?

You need something official which "connects the dots" between the two names.  I'm curious as to how you got a passport in a name other than your BC...

I am in illegitimate child that is why in my BC I have my mother's maiden name as my surname. I was advised to file an adoption but I was thinking that it would take some time to finish the process. So that is why I was thinking of just filing the BC with an affidavit.

All of your documents have to be the same name unless you have court documents proving a legal name change

Not necessarily. In my country you can only alter the birth certificate in case of adoption if approved by court (given name, last name, DOB if necessary etc) but hardly for a teenage or an adult person. Adoption is only allowed for minors.  But an individual getting their first domestic passport can freely choose between their mother's and assumed father's last names. No court involvement needed, it's the privilege granted once and to teenagers only. However I believe the change must be in the Civil Register. So this may be the case with OP
