Based on the details given, IMO the most likely reason he was denied was not based on your relationship, but because they suspect he might be using you to get the green card - ie pretend he is in love

An I-290B is to appeal a denial of a petition with USCIS. This was a refusal of the visa by DOS. It does not apply to the OP's situation.   A refused K-1 due to lack of a bona fide relations

My fiancé visa also got refused last month.. we met 9 times in a year after we got engaged.. I was in Dubai he came all the way from NY.. anyway now we planning to get married and apply CR1.. did you guys get an approval yet?
