My son applied for visa lately, but he is a heavy smoker. I want to know dose a smoking person face any problem in chest X RAY or anything that can lead  to visa rejection?? Because if there is any restrictions, maybe he should quit or at least vape? I have read at Vaping Daily that it is not as dangerous as sigarettes.

Cigarette smoking, in itself, will not be cause for denial.  The xray will be used to rule out evidence of TB.

I think the question is, what is he smoking? LOL

If he is requesting a student visa (the subject of the subforum), there is no medical exam.

While there is no medical exam, the student visa applicant must send in a form signed off by a doctor for certain tests and vaccinations.  My niece had to do it for her student visa.

I'm pretty sure that was asked by the university. I had a student visa and my sister too, and we didn't need any forms from doctors. I do remember filling in some information for the university. What they can require once you arrive to the university is a TB test, that is a regulation from USCIS or DOS if I'm not mistaken.
